Payments/Refunds

Payments must be paid in full unless discussed and agreed upon by a service provider at Medspa DeLuxe LA.  There is a no refund for any purchases however store credit can be granted. Laser treatments packages may be “froze” and kept on hold in the event of a medical emergency or pregnancy. Unless requested by patient upon day of treatment, syringes with additional product will not be kept for later use. Upon request, paid syringes may be stored in house for later use for up to a three month period; if full syringe is not used during initial visit and is stored, a service charge of $50 may be applied to second visit for the remainder of the service with the nurse injector.

Appointments

Services and treatments are available by appointment only. A deposit of $50 is required in order to secure a time on our schedule for most treatments; this deposit goes towards your treatment price. Late cancellation and no show fees apply for all appointments (please see below). Please arrive 10-15 minutes early for appointments to allow time for completing consent forms or other paperwork. If you are running late, as a courtesy, please let us know as soon as possible and we will try to accommodate you without disrupting other clients appointments. If you arrive more than 10 minutes late, we may need to reschedule your appointment.

We will be our best to accommodate requests for a particular technician. However, if your requested technician is not available during the time/day you requested we ask for your cooperation in allowing us to serve you with another technician. 

Follow up appointments are made in the best interest of the patient and the office. Should you state that you do not want a follow up appointment, we leave the patient responsible in informing our office should any concerns or complications arise. 

Dermal fillers done at Medspa DeLuxe LA can be store for two weeks and may be used during the two week follow up appointment, per patient request. Otherwise residual filler will be disposed by office unless stated otherwise. 

Cancellation Policy

Medspa DeLuxe LA has a 48 hour cancellation policy. Should you need to cancel or reschedule your appointment please give us a 48 hour notice so we can make the time available for other clients. There are no exceptions. We accept notice for cancellations and rescheduling appointments via phone, text or voicemail. Please note: our system will automatically text you 48 hours before your appointment in order to provide you with ample time to either reschedule or cancel your appointment. We will also call you 2 days prior, and text one day prior to your appointment day. 

Failure to show up for a scheduled appointment may result in a $50 charge on the card on file last used at our office or will be charged during next appointment. All packages (including laser packages, microneedling, facials and chemical peels); an unused treatment will be forfeited for the late cancel or no show appointment. If you do not supply us with a credit card number and you have a promotion being used for service, we will require that a $35 deposit fee be used to book any further services with us. The deposit fee will be used towards your service.

Photo Consent and Release

I authorize the use and disclosure of my photographs/video images, and/or testimonial is for marketing purposes by Medspa DeLuxe LA. I understand that information disclosed pursuant to this authorization be subject to re-disclosure and may not longer by protected by HIPPA privacy regulations. I also agree and allow a Medspa DeLuxe LA staff member to take photos of my treatment and/or treated areas to be used for the purpose of monitoring my progress. I understand that I may revoke this authorization at any time, but such revocation must be in writing and received by the practice via registered mail. Revocation affects disclosure moving forward and I not retroactive my authorization.
